I'm not sure about that! In Mario Party: the Top 100, Toad mentions he is actually wearing a cap... And he also removes his cap in some episodes in Super Show and Adventures of SMB3... Huh?

Perhaps the hat is still naturally part of their body somehow, since they are the main inhabitants of the Mushroom Kingdom after all.
