"Meat Boy (map pack)". That s*** was ridiculous.
Close runner ups are Chip's Challenge (but that goes from "how can I beat this level" to "why should I beat this level" imo) and the skyscraper warp zone in Super Meat Boy.

Fire Emblem: Thracia 776. That game is absolutely nuts - especially if you want to recruit all playable characters and make sure that everybody survives.
(I'm only counting games that I've actually been able to finish.) Course clear! You got a card.

Hardest game... probably Big Rigs Over the Road Racing. Llke pedigree, the glitches. Also this game had no goal.

Zelda II. I'm not very good at games in general so I find a lot of games hard, but with this one I was unable to play more than 15 minutes into it because I kept getting game overs.

Touhou 15 - Legacy of Lunatic Kingdom (Lunatic difficulty, Ultra mode)
Can't even get to the first boss, even after 1 year of experience playing this franchise of danmaku games. Just... freaking insane... I usually get to this part:

Not sure I understand the question. Most games have their challenges and I'm sure there's plenty I found too difficult to complete for various reasons: too difficult for me to stay interested, too difficult for my hands to move the right way, too difficult for my brain to figure out where to go next, etc. I don't think there's anything I could say is number one above everything else.
Elyk's example of Tetris is great, because while I think I'm decent enough at Tetris and usually "beat" it, endless mode goes on forever and I've never been able to have a game last forever.

Far Cry 1. I've never had such frustrating time with a shooter before or ever since. Bafling health system that contradicted with the level design, game becomes run n gun style after the midway point even though game was clearly designed as a stealth game (you die very fast, enemies take an entire magazine to the chest but one to the head, you aim reliably unless you stay still) and a terrible checkpoint system.
Weapon system is also really badly executed. You can only carry four weapons. In order to swap out a weapon, you need to manually drop it first and then walk over to the weapon you want without touching the weapons you don't want (including the weapon you just dropped).
Alright I'm convinced that this is the absolute maximum amount of ass the Nintendo 64 could render - civvie11

Hard to say. I guess I don't really like a hard challenge because I nope out of games pretty fast if they're beating me up too bad. Probably a shooter or RTS.

So far, the hardest game I ever played was Geometry Dash. Yes, Geometry Dash. The demon-rated levels (that's the hard ones) can throw me off-course with its timings, synchronization, and ways of flying, and I have to start over from the beginning of the level everytime I crash.
I would consider Undertale to be my second hardest game but it's an easier Touhou and it's actually fun. It's a little hard, but Idk.
